Managed Care (HMO, PPO, POS, etc.) patients must present their plan identification cards when registering for each office visit.  Co-payments and charges for non-covered services must be paid at each visit.

For patients not covered by insurance in which PAMPA participates, we require complete payment for our services at the time of the office visit.  Reimbursement may be obtained from your insurer. At the time of your office visit, you will receive a charge slip which explains in detail the charges incurred. Your charge slip is accepted by most insurance companies as the "physician's statement" when you submit your claim to them.  We understand that this may be difficult for families at times; therefore, our patient account representatives will be happy to discuss payment arrangements with you.
